For instance, after the emerge of ’swine’ flu in several countries, Indonesia government has re-allocated budget for bird flu to ’swine’ flu control and prevention.

Madiun, East Java

Madiun, East Java — In 2009, bird flu virus has been spreading in two districts of Madiun, Kecamatan Mangunharjo and Kertoharjo. Meanwhile no case reported yet from Kecamatan Taman. Latest case is reported from Kelurahan Sogaten Kecamatan Manguharjo, this week. About 12 chickens reared by locals found suddenly died and tested positive H5N1 virus.

Bangkalan, Madura

Bangkalan, Madura – Numbers of chickens belong to people in Kecamatan Kamal reported dead. This incidence occurred in three villages, Desa Gili Timur, Telang and Kebun. Ironically, people never report these findings to the authorities.

Based on the rumors about suddenly dead chickens, Participatory Disease Response and Surveillance (PDSR) team investigated the suspected areas and found evidences of dead chickens. Further test solved that chicken deaths were caused by bird flu infection.

Mostly people do not know the symptom of bird flu infection and its threat to human. Usually they eat or sell the sick chickens.

Team of PDRS, during their duty, visits villagers from house-to-house to find and do rapid test to dead chickens. When test shows positive bird flu infection, next procedure is depopulation to chickens and burning chicken cages within 200 meter radius. If people refuse, then quarantine will be done to the area followed by disinfection every morning and afternoon for a week.

 

One of PSDR team, Ali Makki said that recently, bird flu infection doesn’t show any particular signs. Birds will be only found dead without any convulsion or apparent illness. Bird flu infection can be only confirmed by rapid test.

Jember, East Java


Jember, East Java – After previous announcement mentioning 14 districts in Jember have been infected by bird flu virus, now Livestock Service (Disnak) Jember announces 17 districts from 31 districts in Jember have been infected. According to SUdarsono DVM, Head of Animal Health Division Disnak Jember, infected districts are Kecamatan Kalisat, Panti, Jenggawah, Ajung, Pakusari, Sumbersari, Tempurejo, Mumbulsari, Sukorambi, Patrang, Ambulu, Tanggul, Kaliwates, Jelbuk, Arjasa, Sukowono, and Puger.

The spreading of virus into over 17 districts may cause further spreading to other districts. Sudarsono messaged people to maintain flock biosecurity. Suddenly dead chickens should be reported to authorities and diminished by slaughtering. People also informed to to dispose dead cickens to the river, because people used river stream in daily activities.

Mostly people do not have any knowledge about bird flu virus so that they treat dead chickens without biosecurity measures.

Pamekasan, Madura

Pamekasan, Madura, East Java – Total 53 chickens belong to people in Proppo district Pamekasan, Madura reported suddenly died. Laboratory test showed positive bird flu infection.

Tasikmalaya, West Java

Tasikmalaya, West Java – Rapid and laboratory tests showed poultry deaths in Kabupaten Tasikmalaya caused by bird flu virus. The infected areas reported are: Desa Sarimukti, Kecamatan Karangnunggal; Desa Karyabakti, Kecamatan Parungponteng; Desa Buniasih, Kecamatan Pancatengah; and Desa Puteran,Kecamatan Pagerageung.

Livestock, Fishery and Marine Service (DPPK), Municipal Government (Pemkab) Tasikmalaya so far cannot do any further control measures such as area isolation or poultry depopulation because of no budget allocation.

Instead, DPPK Tasikmalaya is doing intensive monitoring and information extension to public about environmental health and hygiene lifestyle; also disinfection and vaccination to chickens.

“The pattern of bird death is different from previous cases. Before, chicken deaths could reach hundreds in total. This time, in average only about 50 chickens found dead, but all of them tested positive. This time it’s impossible to do depopulation and area isolation, since we don’t have budget for compensation”, said Ati Purnamawati, Veterinary Functional DDPK Kabupaten Tasikmalaya.

Menawhile, people are worrying if human infection occurred, because lots of people are rearing birds. People hopes government can take rational actions before anyone get infected.

“Bird flu virus has been spreading already, and has been confirmed by laboratory test. People hopes government can put priority to bird flu eradication budget allocation, before the disease expands farther”, said Head of Municipal Government Tasikmalaya, Uu Ruzhanul Uluum.

The Jember regency administration announced Tuesday the H5N1 bird flu virus had spread to 17 of the regency’s 31 districts after lab tests showed 5,449 chickens that died in those areas were infected by the virus.

The local administration has conducted inspections on the traffic of poultry coming from Banyuwangi, Situbondo and a number of areas supplying poultry to Jember.

The affected districts are: Kalisat, Panti, Jenggawah, Ajung, Pakusari, Sumbersari, Tempurejo, Mumbulsari, Sukorambi, Patrang, Ambulu, Tanggul, Kaliwates, Jelbuk, Arjasa, Sukowono and Puger.

Sudarsono, head of the animal husbandry department at the Livestock, Fishery and Marine Affairs Agency in Jember, said his office had culled 1,000 chickens owned by residents in the affected districts.

“We have also sprayed disinfectant in traditional markets and poultry farms, especially after the provincial administration issued a warning on the global spread of swine flu,” Sudarsono said.

Head of the Jember Health Agency Olong Fajri Maulana said the bird flu infection in the regency was considered highly unexpected because his office had received no reports of poultry infected by the H5N1 virus since 2006.

“It is the first occurrence in Jember and it has spread to quite a large area,” he said.

Meanwhile, in relation to the swine flu outbreak in America and Europe, East Java Husbandry Agency head Sigit Hanggono said his office had carried out a number of anticipatory measures to prevent the spread of the disease in the province, despite the smaller number of pig farms in East Java.

“We have instructed the animal quarantine centers at sea and airports to raise their level of alert. We have also asked the related agencies to disinfect pig farms, such as those in Tulungagung, Sidoarjo, Jombang and Lumajang,” Sigit said.

Based on a survey conducted by the East Java BPS Statistics Agency, the pig population in the province stood at around 75,000 in 2008, a significant rise from 48,000 the previous year.

The poultry population in East Java stands at 98 million, scattered across 38 regencies and cities. East Java Governor Soekarwo said he had proposed the distribution of emergency mitigation funds to the provincial legislature given the vast area of the province.

“We will submit the proposal from the husbandry and health offices to the legislature in the next couple of days,” Soekarwo said.

Head of the Disease Control and Medical Problems division at the provincial health agency Budi Rahaju said such funds were badly needed given the limited supplies of vaccines at the health agency.

“The current stock of Tamiflu, a vaccine for bird and swine flu, at the health agency is insufficient to supply hundreds of community health centers across the province,” Budi said.

Pekan Baru, Riau ::: Outbreaks in chickens

Pekanbaru, Riau – Livestock Service Riau province stated number of chickens killed by bird flu virus showing drastic declining. In 2005, approximately 31,501 chickens died of positive bird flu virus, while in 2008 the number showed 2,897 chickens. In 2009 up to April, 1,150 chickens have been reported died of bird flu infection.

 

Even though the graph shows declining, Livestock Service of Riau will keep continue all control and prevention efforts.

Currently, Livestock Service is empowering 77 personnel for Participatory Disease Surveillance and Response (PDSR) for bird flu monitoring and prevention in Riau.

 

According to the data, Head of Livestock Service Riau – Patrianov – described, in 2009, 374 birds in Siak and 262 birds in Kuansiang were reported positive bird flu infection. Livestock Service appears to enhance its prevention efforts by adding number of PDSR personnel.

“Ideally, for animal bird flu suspect control, we need one personnel for one district. Compare to 150 districts available in Riau, we definitely lack of personnel”, said Patrianov.

Jember, East Java ::: Bird flu outbreaks in chickens

Jember, East Java – Numbers of poultries from 17 districts in Kabupaten jember, East Java have been confirmed positive bird flu infection. District mentioned above are Kalisat, Panti, Jenggawah, Ajung, Pakusari, Jelbuk, Arjasa, Sukuwono and Puger.

 

Head of Animal Health, Livestock and Fishery Service of Jember, Sudarsono DVM, stated that bird flu spreading has expanded within this 4 months. “Poultries from 17 district in jember stated positive bird flu “, said Sudarsono.

 

Most of infected birds are belong to locals. A chicken farm in Kecamatan Pakusari was also reported attacked by bird flu virus. As control measure, the farmer should cull, in total 1,511 chickens.

 

Sudarsono also mentioned they have received instruction letter from The Governor of East Java through Provincial Livestock Office to be aware to bird flu spreading. Separatley, Head of Health Service (Dinkes) Jember, dr Olong Fajiri Maulana stated his side hasn’t receive any information about bird flu outbreak in Jember.

 

“I believe there is no bird flu human suspect in Kabupaten Jember”, said Olong.

Situbondo, East Java ::: Bird flu outbreak in chickens.

Situbondo – Three districts (kecamatan) in Tapal Kuda announced positive bird flu infection according to Livestock Service of Situbondo.

Approximately 200 chickens belong to people in Dusun Lembung, Desa Kandang, Kecamatan Kapongan, suddenly died.

All dead chickens appeared bluish and swollen, accompanied with mucus discharge from mouth.

Livestock service has confirmed the dead chickens were infected by avian influenza virus by rapid test.

Control measurements have been done by disinfection and burning the dead chickens.

Head of Animal Health Division, Pipin Arimbi said that bird flu virus circulating in Dusun Lembung is the largest in Situbondo because of its rapid spreading.

Desa Kandang, Kecamatan Kapongan is the fourth location which has been stated positive bird flu. Other areas are Desa Sumberrejo and Desa Sumberwaru Kecamatan Asembagus, Kelurahan Mimbaan, Kecamatan Panji and Desa Kotakan Kecamatan Kota.

Up to now, no human infection reported.

Bird flu kills 40 chickens in Madura


Pamekasan, E Java (ANTARA News) – At least 40 chickens died a sudden death on Monday for bird flu infections in Proppo subditrict, Pamekasan, Madura district East Java, an animal husbandry official said.

We have conducted a check on the dead chickens and it is true that they were infected with bird flu virus,” Hadarwati, head of the animal health affairs of the Animal Husbandry Service Office of Pamekasan said here on Monday.

At least 40 chickens belonging to Sayfei died on Monday. Syafei and other residents suspected the death of the chickens was due to bird flue so that they reported to the local animal husbandry office.

“I feared that my chickens were infected with the bird flu virus,” Syafei said.

The Pamekasan Animal Husbandry office immediately take the dead chicken samples and checked them, and found that it was positive to have been infected with the virus.(*)

Padang, West Sumatera ::: Bird flu suspect recovering

Dharmasraya, Padang – Legiman (50) had been hospitalized at Sungai Rumbai public health center (Puskemas) for two days. Dozens of chickens belong to the patient – were reported died. One of Sungai Rumbai authorities said the disease has attacked several places in Dharmasraya. Few days ago another bird flu suspect patient found at Kenagarian Timpes, Kecamatan Timpes.

 

Meanwhile, Legiman is reported improving and allowed to return home.

Head of Health Service Dharmasraya, dr leli Yarma confirmed the sudden death in chickens in that area. Livestock and Health Service have taken control measures by burning the dead chickens. Leli also mentioned that Legiman – the owner of chickens – had been treated as bird flu suspect.

Ponorogo :  More on three bird flu suspect patients

Ponorogo – A Father, his son, and their neighbor are suspected to contract bird flu virus. They developed the same symptoms like high fever up to 38, breathing difficulty, coughing and common flu symptoms. The patients had contact history with dozens of suddenly dead chickens around their house. 

The patients – Prayoga Dita Saputra, 4 (son); Kateno, 37 (father); and Suwandi, 35 (neighbor) – residents of Dusun Plosorejo, Desa Kemiri, Kecamatan Jenangan, Kabupaten Ponorogo, are now treated at public health center (Puskesmas) Jenangan, Kabupaten Ponorogo.  A health worker from the Public Health Center, Wiwik Suprihatin, said the patients showed weakness, high fever, breathing difficulties and flu symptoms. The medical team suspected patients may suffer of bird flu infection. 

“It’s still uncertain whether they have contracted the bird flu virus or not. We’re still waiting for the laboratory confirmation. However, acccording to the symptoms, they’re stated as bird flu suspect patients”, said Wiwik.  The patients, Kateno, said many chickens died around their neighborhood. “Actually, our family is not the only sufferer. Other people also developed similar symptoms. But they haven’t checked their health at the Public Health Center”, said Kateno.

The Head of the Public Health Center of Jenangan, dr Siti Nurfaidah, explained that initial diagnosis showed H5N1 (bird flu) spreading signs of influenza-like illness. “We have given Tamiflu to the patients”, added Siti.  Blood samples have been sent to Hardjono hospital laboratory, Kabupaten Ponorogo. The patients supposed to receive chest x-ray, but it was postponed because of the high cost.  “Within this week, there have been 21 patients treated came from the same village. However, these three patients are the most critical. In general, we have treated the patients with anti-influenza drugs for initial prevention”, stated Siti.

Government hopes MTA of bird flu virus specimen will be established soon.

Makassar, South Sulawesi – Government hopes the Material Transfer Agreement (MTA) of bird flu virus specimen will be established on inter-government meeting of WHO member countries in Geneva, Switzerland next May.

“We will do our maximum effort to achieve agreement.  Up to the present moment the dialogue is still tough”, said Public Health Division Special Staff of Health Minister, Widjaja Lukito while the Simulation of Bird Flu Pandemic Epicenter was ongoing in Kelurahan Kassi Kassi, Makassar, South Sulawesi, Saturday (25/4).

Currently, government is still assessing the content of MTA draft and doing preparation for WHO members inter-government meeting which planned to be held on 15-16 May.

Widjaja mentioned the agreement of benefit shares for material or biological specimen exchange has been noted in introduction of MTA standard draft. However, Indonesia government is hoping this subject will be described more detail to include the rights and obligations of related parties.

“We want this subject is also described in the stem of agreement. Therefore, that any details about rights and obligations of each party are also mentioned. Acknowledgement to those points has not been achieved until now”, added Widjaja.

Indonesia government desires the rights and obligations of each party can be legalized – by meaning to engage the contributor and recipient of specimen by law. This may establish a fair and transparent specimen exchange.

As well as other developing countries, Indonesia requires a benefit sharing such as vaccine contribution.

“Developing countries which are impacted and providing specimens should be able to buy vaccine with lower cost and prioritized. Therefore, far, we are having difficulty getting vaccine because we have to compete with developed countries”, said Widjaja. Furthermore, up to present moment, vaccine purchasing is done beyond a purchasing deal. This mechanism is complicating for developing countries due to limiting budget.

Indonesia and other developing countries governments will fight at the forum so that local researchers can participate in active in bird flu vaccine development and therapeutics research.

“Clearly, Indonesia wants this MTA of bird flu specimen exchange can be achieved as a simple agreement but can serve the needs of developing countries”, said Widjaja.

Makassar, South Sulawesi – Currently, bird flu virus is transmitted from animals. What will happen if transmission has been occurred among human in urban neighborhood?

Indonesia Ministry of Heath introduced this information through bird flu virus transmission control simulation at Makassar, South Sulawesi.

“Hopefully by this event we can provide knowledge to public about prevention of influenza pandemic epicenter”, said General Director of Disease Control and Environmental Health (P2PL), Tjandra Yoga in Makassar, Saturday (25/4).

Pandemic happens when bird flu transmission has occurred among human in broad and continuing terms. This seems has potential to be occurred in Indonesia, in particular several human- to-human cases among family members have been occurred.

“This virus has alarmed the world and we don’t know when this will happen”, added Tjandra.

This simulation has spent about 4 billions rupiah and been funded by Indonesia country spending plan budget (APBN), WHO and CDC Atlanta. Current event in Makassar is a sequel of similar event in Jembrana Bali last year.

The Simulation

Taking (63), resident of Kassi-Kassi, Makassar has been referred to local public health center due to breathing difficulty and coughing. Further diagnosis showed Taking has been infected by bird flu virus. He was then referred to Wahidin Sudiro Husodo hospital.

The next day one of the paramedic developed fever and suspected to contract bird flu virus. This had alarmed local health service and Ministry of Health (Depkes).

Taking’s house is quarantined for unlimited term. As prevention measures, local government and Depkes isolated family members to stop possible human transmission.

As information, this incidence is a simulation of most threatens level of bird flu infection. “We hope public will understand and be readily to respond if similar case happens in future”, said General Director of Disease Control and Environmental Health (P2PL), Tjandra Yoga.
