Jakarta – Two bird flu victims had been tested H5N1 positive. A residents of Bekasi, West Java province, named NL, 25 years old, tested positive bird flu H5N1 infection. NL had died on 25 January 2010. Laboratory test showed positive H5N1 infection, said Tritarayati, the Head of Public Communication Center Ministry of Health. Victim was possibly infected from direct contact with birds.

Another victim, AD, 3 years old, a resident of Jakarta, was reported to show mild influenza like illness signs. “Laboratory test showed positive H5N1 infection. Patient has been recovered and not showing any of ILI signs,” said Tritarayati.

Indonesia ::: Govt says bird flu has been cleared

Jakarta – Indonesia has successfully controlled bird flu H5N1. This is proved by the declining of death rate caused by this deadly virus during 2009 to the beginning of 2010.

World Health Organization (WHO) previously recorded Indonesia at the top list of H5N1 human infection. Until 2009, 134 of 161 total cases had died of H5N1 in Indonesia

For novel H1N1 2009 flu, total 1,097 cases reported in Indonesia with 10 deaths.

According to dr Iwan M Muljono of Ministry of Health, bird flu case in Indonesia was relatively stable and approaching normal state.

 “Bird flu case has been cleared but Ministry of Health will continue public socialization to prevent any possible re-emerge,” said Iwan in a seminar with title Preparing Indonesia for Influenza Pandemic, held at Hotel Le Meridien, Jalan Jendral Sudirman, South Jakarta, on Tuesday (9/2).

Meanwhile, Director of Basic Medical Care Ministry of Health, Marwan Nuftri said number of fatality caused by H5N1 until 2010 had reached 80 percent.

 “Over 150 cases have been reported with death over 100 people. But death rate has declined because we attempted control such as socialization, strengthen of health practitioner and public health center,” said Marwan.

Marwan also mentioned pandemic issue was a crucial international situation and strict control of communication and transportation which might become the source of infection, was needed.

“It means any source of infection should be considerably monitored. Indonesia has large population and high death rate is found in particular in West Java and North Sumatera,” added Marwan.

West Java ::: H5N1 outbreaks in birds

Bandung – Bird flu/avian influenza/AI H5N1 case has reached over a half of total cases recorded in West Java on 2009. At the beginning of 2010, about 70 bird flu cases had been found in ten municipals (kabupaten). Head of Livestock Service of West Java, Koesmayadi Tatang Padmadinata stated that current situation had been a warning.

So far, Livestock Service found total 2,695 native/backyard chickens deaths distributed within 52 sub-districts (kecamatan) in 10 municipals, Cirebon, Majalengka, Kuningan, Indramayu, Bandung, Bandung Barat, Sumedang, Garut, Tasikmalaya and Ciamis.

During 2009, there were 170 bird flu cases, which had killed 5,528 backyard chickens in 21 municipals/cities. Even though, four cities did not report any bird flu case, they were Cirebon, Bekasi, Sukabumi and Banjar.

The increase of cases was suggested because of the change of diagnostic method. The Public Health Division of Livestock Service of West Java, Sri Mudjiatiningsih mentioned that the procedural change had been an agreement between FAO (who managed the Participatory Diseases Surveillance and Response) with Livestock Services.  Previously, bird flu control measures would be done if only molecular biology or PCR test showed positive of AI to the samples which had been tested positive by rapid test. However, new policy of diagnostic method would be done by applying every bird flu control measures when rapid test showed positive. Control measures here would be biosecurity procedure and vaccination. Depopulation would be done only when PCR showed positive, described Sri.

Until the present moment, Livestock Service of West Java had confirmed that 3 of 70 positive rapid test cases were positive by PCR. Those samples came from Kabupaten Bandung Barat: Desa Karyawangi Parongpong and Desa Cipada Kecamatan Cisarua; and from Pasirwangi, Kabupaten Garut. Test was not completely representing the outbreak because some affected area did not send the positive rapid test samples to be confirmed by PCR. Also, some outbreak samples were positive for Newcastle Disease (ND) instead AI infection.

In 2010, Livestock Service of West Java to prepare 3,245 millions of bird flu vaccine dose for birds including 245 thousand of doses from last year stock. A total of 6,858 million of birds were vaccinated last year, in details they were 771.5% backyard/native chickens, 11.72% ducks, 5.21% layer chickens, 4.42% broiler chickens, and the rest were swan, muscovy ducks and birds.

Pekanbaru, Riau ::: Prevalence increases up to six hundred percent

Pekanbaru – About 2,751 chickens had died of bird flu virus within previous month in Riau. This has folded the number of bird flu cases in chicken by 600 percent compare to number at same period of last year outbreak, 441 chickens.

Head of Livestock Service of Riau Province, Patrianov said massive chickens deaths had been reported from six municipals/cities in Riau: Kampar 1,995 chickens; Siak 351, Indragiri Hilir 234, Indragiri Hulu 128, Rokan Hulu 22, and Pekanbaru 14. The shift of rainy season had influenced the replication of the virus. Several areas in Riau were still drowned by flood which had increased the rate of virus spread, added Patrianov.

In the days of flood, people had trouble to diminish or bury dead chickens, so they preferred to dispose the chicken’s body to flowing water or puddle around their residences.

According to Patrianov, most severe bird flu attack in Riau had occurred in broiler chickens in Kabupaten Kampar with total case up to 1,900. Remaining cases were reported in backyard chickens that reared by locals.

Indragiri Hulu has been the most susceptible area of bird flu transmission from birds to human. At the end of 2009, an 8-year-old boy from Kecamatan Kelayang had died of bird flu virus. Within the former month, three more children from the same sub-districts (kecamatan) were suspected of having bird flu infection, named Julian Iman Jaya (3.5 years old), Annisa (4.5 years old) and Rafli (6.5 months old). They have been treated at Arifin Achmad regional hospital since the former few days to a week.

South Jakarta ::: Official conducts operation of healthy chicken transport

Kebayoran Lama – Livestock and Fishery Sub-service of South Jakarta stopped about 17 from 26 units of truck to transport chicken meat from outside Jakarta. Those chicken meat trucks could not show animal health certificate.

This action, Healthy Chicken Transport Operation (Operasi Transportasi Ayam Sehat/OTAS), was carried out at Jl. Raya Kebayoran Lama and Jl. Ciledug Raya, Kebayoran Lama, in particular to trucks carrying chicken carcass to be delivered to traditional markets, said official of Livvestock and Fishery Service, South Jakarta, Chaidir Taufik.

Total bird carcasses loaded in those 26 units of trucks were about 19,180 chickens and 800 ducks, which were coming from Subang, Bogor, Serpong and Sukabumi.

OTAS action was done to anticipate bird flu outbreak in Jakarta, in particular South Jakarta.

Chaidir also mentioned, OTAS action was done regarding to the death of bird flu suspect patient, Nurlalila, a resident of Jl. Pengadegan Barat V, on 26 January 2010 in Pasar Rebo hospital.

Information mentioned that victim brought four chickens from Cibitung Tangerang on the former month. One chicken was slaughtered and then victim developed sickness at the following days. Victim was then treated and tested positive bird flu infection. The remaining three chickens were returned back to Cibitung.

Pekanbaru, Riau ::: Two babies suspected of having bird flu

Pekanbaru – Arifin Achmad regional hospital in Pekanbaru received two more bird flu (H5N1) suspect patients from Kabupaten Indragiri Hulu, Riau Province.

Those patients were AN, six months old, and YL, three years and six months old. They both referred to Arifin Achmad hospital on Thursday afternoon and evening.

Both patients were neighbors, resided in Kecamatan Kelayang, Indragiri Hulu. They had contact history with dead chickens.

Patients are now under intensive treatment.

Wawan, AN’s parent said his child had been treated for two days at home, but the fever was increasing up to 38℃ so that he rushed the six-month-old to Indrasari hospital in Rengat. The baby was treated for two hours in that hospital before then transferred to Arifin Achmad hospital because doctors suspected bird flu infection.

Meanwhile, Iwan, uncle of YL stated similar information. Before been admitted to Arifin Achmad hospital, YL’s parents mentioned that their son was playing with quail two days before development of fever. He also mentioned that many backyard chickens around YL’s resident died of unknown cause. However, people took it as a usual event.

In mean time, officials have not done any control action, neither examining surrounding people’s health status nor disinfecting suspected area, even though at the end of last year the area was stated as bird flu endemic.

At the end of December 2009, Arifin Achmad hospital received two bird flu patients from Kabupaten Indragiri Hulu, where one of them, Ananda (8) resident of Dusun Tuo Pelang, Kecamatan Kelayang had died.

Another victim, An Nisha (4), resident of Desa Bongkal Malang, Kecamatan Simpang Kelayang had been recovered after received intensive treatment.

Purbalingga, Central Java ::: Thousands of layer chicken positively die of bird flu infection

Purbalingga – Avian Influenza (AI) or bird flu killed thousands of layer chickens in Desa Tegalpingen and Tumanggal, Kecamatan Pengadegan, Purbalingga, Central Java. It had been two weeks where people were found to burying and burning bird flu infected birds.

Total number of culled chickens within these 2 weeks had reached 5,500 layer chickens within the productive age, 16-18 months. “Each day I burn about 50-60 dead layer chickens,” admitted a local of Desa Tegalpingen who had lost about 500 chickens within these couple of weeks.

Depopulation was done by burning the dead chicken to prevent possible transmission to other farms because Pengadegan is one of layer chicken central.

Head of Livestock and Fishery Service (DIsnakkan) of Kabupaten Purbalingga, Suhartono confirmed that those chickens had died of bird flu infection.Prompt action had been taken by localizing bird flu affected farm areas to be disinfected and vaccinated. “Until now, we have vaccinate about 21 thousand of chickens. “We will receive more bird flu vaccine support from government, about 100 doses, ,” said Suhartono.

“We advised people to enhance the bio-security and vaccination practice to minimize loss,” said Suhartono.

Pengadegan is one of layer chicken centrals, consist of 21 local farms with over 50,000 chickens population.

Indramayu, West Java ::: Bird flu attacks chickens and birds

Indramayu – Hundreds of chicken and birds suddenly died in Desa Pasekan and Brondong, Kecamatan Pasekan, Kabupaten Indramayu. Died chickens showed discharge from nose and bluish carcass. One of the farmers testified that he found his chickens had died in the morning whereas they appeared healthy on the previous day.

Officials have done control measures by burying dead chickens and spraying disinfection to chicken’s houses.

Official of Agriculture and Livestock Service of Kabupaten Indramayu, Nanang Hidayat, said that bird flu outbreak tended to occur during rainy season. He also expected the outbreak would continue until next March. Because chickens were reared freely, once outbreak emerged, the virus would circulate rapidly and transmit disease to other chicken, added Nanang.

Karanganyar, Central Java ::: Officials collect bird samples from deceased bird flu suspect’s neighborhood

Karanganyar – Livestock and Fishery Service of Kabupaten Karanganyar conducted investigation to chicken samples such as cloacal and throat swab of birds in Desa Wonorejo, Jatiyoso, regarding to the death of bird flu suspect patient named WD, 22 years old.

Head of Animal Health Division, Saptawan Hadiputra mentioned officials had collected about 25 chicken samples to be subjected for laboratory test, since they did not find any bird flu evidence by field investigation.

Victim is a meatball seller (meatball is bakso in Bahasa Indonesia. Typically, sellers are circulating with their cart to get costumers. -adm-), and suggested to have caught infection around his place in Cirebon.  For this evidence, Livestock and Fishery Service of Kabupaten Karanganyar, Central Java, had contacted its counterpart in Kabupaten Cirebon, West Java for more investigation.

Wd, resident of Desa Wonorejo, Jatiyoso, possibly died of bird flu infection on Wednesday (20/1). Patient finally died after refused to be transferred from Wonogiri regional hospital to Dr Moewardi regional hospital to get medical treatment.

Cirebon, West Java; Rembang, Central Java ::: H5N1 outbreak in birds

Outbreak report from Cirebon, West Java. H5N1 seems to surge in Cirebon. Officials linked this incident with two suspected human cases, a 41-year-old and deceased 22-year-old.

Other report mentioned H5N1 in Kabupaten Rembang, also in birds.-adm-

Paliaman – Animal Health Center in Kaliwedi, Kabupaten Cirebon found bird flu H5N1 had been spreading to four villages in three sub-districts, Desa Ciawi, Kecamatan Paliaman, Desa Danawinangun and Desa Pekantingan, Kecamatan Klangenan and Desa Kedungsana, Kecamatan Plumbon.

Head of Animal Health Center Kaliwedi, Suharto said the virus currently transmitted among animals. Until now, no positive case in human found from those three sub-districts.

Meanwhile, Head of Health Service, Rinny R. Sechan confirmed that a resident of Keluarahan Tukmudal, Kecamatan Sumber, named Duramham Suherman (DS) had been treated in isolation unit of Gunung Jati regional hospital because of suspected of contracting bird flu (H5N1) infection.

DS had fever after contact with suddenly dead chickens before been admitted to hospital. Until now officials are still waiting blood test result for bird flu confirmation of DS.

Rembang – Eight chickens in Kelurahan Magersari, Kecamatan Rembang, died of bird flu attack. Confirmation of chicken samples was done by Agriculture and Forestry Service, Livestock Division of Kabupaten Rembang.

“Test was positive. Fortunately, there was no recurrence case in birds,” said chieftain of Magersari.

Spokesman of Livestock Service of Kabupaten Rembang, Derevmeti Nursiswati, said it was the second case in Kabupaten Rembang until January 2010.

First bird flu case was reported from Desa Sendangmulyo, Kecamatan Gunem, about a week ago before current case.
